CarePlus
Overview
CarePlus provides funding for patients with high health needs. Patients identified as eligible for CarePlus receive four funded visits a year with the General Practice Team.
Patients enrolled in CarePlus will have an initial assessment and three reviews throughout the year which focus on developing a plan for their care and setting goals that will improve the management of their health.
CarePlus is available to any patient that meets the Ministry of Health criteria for the programme and does not have a High Use Health Card (HUHC).

- Telephone advice on CarePlus issues
