Upon confirmation of registration your payment can be made by cheque or by direct credit as detailed below:
Cheque Payments to be made payable to: South Link Health Post your cheque with a printout of your registration form to: South Link Health, PO Box 6032, Dunedin 9059
Direct Credit to be paid to bank account number: 12-3150-0014735-00 quoting your surname and initials as reference (lease ensure surname and inititals are quoted otherwise your payment will not be identified).
Travel
Road Travel There are set travel reimbursement rates for those travelling by car to the conference. A Travel/Accommodation Reimbursement Form will be in your registration pack for you to complete and either leave at the Registration Desk or fax back to SLH for your reimbursement.
The set travel reimbursement rates are (Otago refers to north of Palmerston and south of Milton):

Air Travel Once your registration has been accepted please arrange your own flights to the conference. A Travel/Accommodation Reimbursement Form will be in your registration pack for you to complete and either leave at the Registration Desk or fax back to SLH for your reimbursement.
Accommodation can be arranged at St Margaret's College on Friday 18 November (at your own cost) & Saturday 19 November 2011 (paid by SLH).
Accommodation at St Margaret's College includes room and breakfast. Rooms are single, with a limited number of twin rooms available. South Link Health will provide Saturday night accommodation only. Additional nights will be at your own cost of $75.00 (incl GST) per night. Partners will be charged $75.00 per night. All additional charges are at delegates' own cost and must be paid in full on check-out (payment is by credit card or cash).
Alternatively you could book your own accommodation and receive $75.00 (incl GST) reimbursement for your Saturday night's accommodation. Please use the Travel/Accommodation Reimbursement Form in your registration pack including a copy of your receipt.
If you choose to book your own accommodation, $75 (incl GST) will be reimbursed by using the Travel/Accommodation Reimbursement form in your registration pack.
Airport Shuttle Transfers
Airport group shuttle transfers can be arranged with Southern Taxi's who have a special conference price of $15-$20 per person each way (dependent on numbers). To arrange airport shuttle transfers phone Southern Taxi's on 0800 829428 or
email info@southerntaxis.co.nz. You will need to provide your name, flight number arrival time/flight number departure time and cellphone number. Southern Taxi's accept Eftpos, Credit Card and of course cash.
Note: if you have not pre-booked a shuttle the price will be $30 each way
